Supported Housing

Supported housing is designed for adults with diverse physical abilities who want to live as independently as possible while knowing that support is available when needed. Homes are staffed by 24/7 care workers, with a nurse providing oversight and care planning. This model is ideal for adults who are too young to move into long-term care, but need more than occasional home visits. Whether in a group community setting or the privacy of your own home, you can have the peace of mind that care is there when you need it.

In this model, you direct your own routines and handle most daily tasks yourself. When assistance is required, whether for personal care, mobility, or everyday activities, it can be provided on a scheduled or as-needed basis. Support is flexible and adapts as needs change, helping you remain in your home for as long as it is safe and appropriate.

Our model is person-centred and based on a social model of care, recognizing that your abilities, routines, and preferences are unique. You are encouraged to direct your own routines and participate fully in daily life and the broader community.

You, along with your family and care team, will be involved in developing a Comprehensive Support Plan. This plan addresses personal goals, physical and health needs, and opportunities for community participation. Support is offered in a way that promotes independence, privacy, and dignity, while ensuring that assistance is available when needed.

Supported housing is for adults who want to live as independently as possible but need more help than can be provided through occasional home visits. You might consider it if you or someone you love:

  • Lives with complex physical limitations or acquired brain injury
  • Needs a 24-hour supervised setting with daily personal care
  • Can make choices about their care by expressing needs, setting goals, and taking part in community life
  • Does not have active mental health or substance concerns
In your new home, you will find:
  • A variety of home settings—from private, fully accessible apartments to shared community homes
  • 24/7 on-site care workers
  • Nurses available for oversight and support
  • Scheduled or as-needed personal assistance for daily living activities
  • Coordination with healthcare providers and case managers
  • Emergency response systems and safety protocols
  • Opportunities for recreation, wellness, and community connection
Although you are encouraged to live as independently as possible and prepare your own meals when able, nutritious meals are also prepared for you, with menu choices that reflect the preferences of your community. Two options are typically available at each meal, and special menus are served for holidays and celebrations. Special diets and texture modifications are available to meet your health needs, with guidance from our Registered Dietitian.

In your community, you will find:

  • Case management and ongoing care planning
  • Nursing oversight
  • Access to rehabilitation services such as physiotherapy and occupational therapy
  • Specialist referrals as needed
  • Support and maintenance for mobility and assistive devices

Activities vary by site and may include:

  • Wellness and fitness programs
  • Creative and recreation opportunities
  • Social gatherings and seasonal celebrations
  • Outings and community events
The cost of supported housing can vary depending on the home you move into. Some homes are publicly subsidized with monthly rates set by BC Ministry of Health guidelines and based on after-tax income. Other homes may require you to be a BC Housing client and follow their rent guidelines. Additional costs may apply for personal items, activities, or optional services. It is best to contact the home directly to learn about specific rates and eligibility requirements.
All move-ins are managed through Vancouver Coastal Health (VCH). To begin, you or your family should contact VCH Home and Community Care Services – Central Intake at 604-263-7377. A care coordinator will arrange an assessment to determine your eligibility and priority for placement.
